Find commonly requested Curie Preschool enrollment, health, school-year, and program documents below. If you are unsure which forms apply to your child, please contact your Curie Preschool campus for assistance.
Most forms are fillable PDFs — you can type directly on screen, sign digitally, and email them back without printing a single page.
Some documents may reference The Madame Curie School of Science & Technology, our former name. These remain valid — the school, ownership, and licensing are unchanged.
How to Complete These Forms
Download and open
Open the PDF in Adobe Acrobat Reader, Preview on a Mac, Microsoft Edge, or the Files app on iPhone and Android. Click any shaded box and type. Press Tab to move to the next box.
Sign it
Type your full name, or draw your signature with a finger or mouse, or insert a photo of your signature. In Adobe Acrobat Reader use Sign → Add Signature. On a Mac use Markup → Sign. On iPhone open the file in Files, tap Markup, then the + button and Signature.
Save it— don't skip this step
Choose File → Save As and save the completed form to your device. Do not use Print, and do not email the form directly from a browser tab — your typing may not be saved. Rename the file as ChildLastName_ChildFirstName_FormName.pdf
Email it
Send completed forms to [email protected] for the Herndon campus or [email protected] for South Riding. You are also welcome to hand any form in at the front desk instead.
